The Dispensary
Living in a rural area we are able to offer dispensing services to all our patients who live more than one mile from a chemist.

Telephone Consultations
All the doctors are happy to speak to you on the telephone, however they may be consulting with another patient when you ring in which case the receptionist will take your number and the doctor will call you back. The practice nurses are also happy to give you advice either personally or on the telephone.

Fit to Work
It is not necessary for the Doctor to issue a certificate for the first week of illness. Self-certificates are available on request from your employer. If you are ill for more than 7 days you will need to see the Doctor for a certificate. From 6th April 2010 you do not need a 'final' certificate to return to work.

Summary Care Records
Consists of an upload of medications, allergies, and adverse reactions from GP records onto a central 'spine' or national database. The SCR may be expanded to included further medical information in time. The Government has decided that patients' records will be uploaded unless they opt out. Loxwood Medical Practice does not offer guidance either way but Opt-out forms are available from reception as well as on the internet at www.connectingforhealth.nhs.uk.

Complaints Information
We aim to provide the best service possible at all times. There may be occasions when you feel this does not happen and if you wish to make a complaint please visit, phone or write to our Practice Manager.

We believe it is important to deal with complaints quickly so you will normally be offered an opportunity to discuss your complaint within 48 hours. You may bring a friend or relative with you.

Please note we have to respect our duty of confidentiality to patients and a patient's written consent is necessary if a complaint is not made by that patient in person.

In addition the Independent Complaints Advocacy Service can give you independent advice and assistance with your complaint tel: 0845 6008616. You are also able to contact PALS on tel: 01903 505456 who will also be able to offer advice.

If you change your name & address
please remember to inform the surgery including your telephone number and postcode by either telephone or by using the Patient Changes section.
