Job title:              Social Prescriber employed by Chichester District Council

2 x Full time 37hrs a week

1 x 28hrs days negotiable across the week

 

Fixed term until 31st March 2024

 

Salary: Full time salary £22,627 - £24,491 (depending on experience)

 

We are looking to expand our Social Prescribing Team in Chichester with 3 new posts. The Social Prescribing role is an exciting opportunity to offer a hollistic approach to supporting an individual’s health and wellbeing. The service provides a well established referral pathway for GPs and other Healthcare professionals, to refer patients with social, emotional or practical needs to a range of non-clinical services. This is based on the belief that the best way to tackle around 70-80% of health issues lies in non-medical solutions.

 

This role will enable you to demonstrate your experience of working face to face with clients along with a range of interpersonal and organisational skills. Using behaviour change techniques, you will work with individuals to set goals and priorities, which will lead to improvments in their health and wellbeing. You will be required to meet the fast paced demands of the service, whilst also providing your clients with a unique and meaningful experience, (training will be provided).

 

As a lone worker you will be encouraged to integrate yourself within your allocated surgeries but will have the support from the wider Social Prescribing team who have been working across the area over the past 2.5 years. Chichester District Council will be the employing organisation and the post holder will be part of the Communities  and Wellbeing division of the council.

 

Under the current circumstances the Social Prescribing Team are working from home working with clients on the phone or via video links.
