Travel Vaccinations
If you intend travelling abroad our practice nurses will be happy to give advice on any vaccinations that might be needed.
In order that they might give the most relevant information, based on the latest guidance, they need information on your itinerary.

Travel form
This completed form, one for each traveller, should be returned to the surgery. Please allow 4 working days for the nurses to review your itinerary and then telephone the surgery for their recommendations.
The receptionists will advise you of the vaccinations required and book appointments for you. It is advisable for certain vaccinations to be spread over a period, with gaps, rather than being administered all at one time. For this reason, please contact the surgery for advice as soon as your plans are known.
Please be aware that some vaccinations are not provided by the NHS and a charge will be made.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
